Key Updates to Forms C-300.5 and C-312.5

The Workers’ Compensation Board has rolled out new versions of Forms C-300.5 and C-312.5, and they’re now the official standard. These updates bring a few important changes that affect how cases are documented and processed.

Here’s what’s different:

  • Representation details: Both forms now ask for information about who represents the claimant, whether there’s been a change in representation, and any related fee agreements.

  • Appeal status: A new field allows users to confirm if an appeal is pending.

  • RB-89 updates (C-312.5 only): Form C-312.5 now requires the date Form RB-89 was filed. That filing will be treated as withdrawn once the Board’s decision is finalized.

What you need to know:

Starting Monday, July 7, 2025, the Board will no longer accept older versions of these forms. Make sure your files are up to date to keep cases moving without disruption.

Translated versions of the updated forms will be made available on the Board’s website as soon as they are ready.
